For example, skeletons might have a higher drop rate for gloves, so if you're looking for a glove, you'd farm content that has lots of skeletons. They also indicated that certain mob types have different chances of dropping different gear types. Apparently hitting certain milestones within the game will unlock higher difficulty levels too, so the loop should look something like farming dungeons for gear so you can beat a boss or complete some sort of event/test/trial that unlocks a new difficulty.
